Enhance your Onboarding Flow with the Nutrition Intake Form to collect essential dietary information from clients, which can then be used to generate personalized meal plans.
How to add the Nutrition Intake Form
Click the "Choose Form" button.
In the pop-up that appears, select the Nutrition Intake Form from the available options.
Within the form, you have the flexibility to choose which optional questions are included or excluded based on your preferences or requirements.
Once you have set up the questions according to your preferences, click the "Add Form" button.
How to use form responses for meal plan generation
By default, the responses from the Nutrition Intake Form will be used to auto-generate a personalized meal plan for the client in the following week. The meals are tailored to align with all macro targets.
To adjust settings, click the Settings icon or the “Auto-generate meal plan” label on the form.
In the Settings pop-up, you can choose to focus the meal plan generation on either All Macros, Calories Only, or Protein and Calories.
If you prefer not to use form responses for meal plan generation, you can disable this option. To do so, click the Settings icon on the form and toggle off the auto-generation feature.
Preview the form
To see what your clients will view on their app, simply tap the "Preview" button. This will display the mobile version of the form, allowing you to review it before assigning it to your client.
What your clients will see
Clients will receive the Nutrition Intake Form when they log into the mobile app for the first time.
After tapping "Let's Start", they will be guided through introductory screens explaining the purpose of the form before proceeding to the questionnaire.
Once they submit the form, their responses will automatically update their profile data. If the Meal Plan Generation setting is enabled, their answers will also be used to create a personalized meal plan.
